Situation

What is your Situation and Question?
  1. Breaking News: Jane fell and broke her hip.
  2. The long term caregiver Sally Jane failed to notice changes in Jane’s mood and behaviors over the last 2 days which led to Jane breaking her hip.

d)    Conversations
i) Nurse Ratchet (Nurse ): Tell me Sally (caregiver) . Did you notice any changes in Jane’s (PLWD) her mood or behavior recently? 
ii) Sally  (Caregiver): I noticed the last 2 days Jane (PLWD) has not been herself. She was more confused than usual, very irritable and she seemed to be very restless. She was so confused she thought the toilet was her chair and she fell off the toilet. 
iii) Nurse Ratchet (Nurse) You have been her caregiver for 2 years and  if you noticed a sudden change from her norm, why didn’t you report it to me?

Oh how do we get caregivers to pay attention to certain changes in mood, cognition, and behaviors in people but they take care of?

Identifying acute delirium in persons living with dementia (PLWD)
